Here it is opened up. She used the Base for the Pop Up cards and scored it at 3 1/2" from each end.  Then she scored at 5 1/2" and fastened the center together just at the ends so that the check, money, or gift certificate would fit inside the indentation punched out with the Word Window Punch.
